| | | | | |  | | | | How Hip Resurfacing is performed Hip Resurfacing, or Surface Replacement Arthroplasty, is a bone-saving and effective alternative to conventional Total Hip Replacement Surgery. This procedure, recently approved by the FDA, will allow a patient to lead a perfectly normal lifestyle, as compared to the restrictions that are usually imposed after Hip Replacement Surgery. Unlike Total Hip Replacement Surgery, Hip Resurfacing doesn't involve the removal of the femoral head and neck; the femoral head, neck, and femur bone are all preserved. The current generation of hip resurfacing devices uses a metal/metal bearing. In a typical hip replacement operation, a metal/polyurethane device is used. The device goes through the femur and into the bone. Compared to metal/polyethylene bearings, resurfaced bearings have demonstrated a much higher level of wear resistance, along with reduced bone loss and less inflammatory tissue reaction of the hip joint. Hip resurfacing is anatomically and bio-mechanically much more similar to the natural hip joint, resulting in increased stability, flexibility, and range of motion. The risk of dislocation is virtually eliminated. Typically, higher activity levels are achieved with less risk. These benefits are possible because the femoral head diameter after the hip resurfacing is very similar to the patient's normal femoral head diameter. Hip resurfacing surgery is also less invasive. The incision required for this 45-minute surgery is usually 6 to 9 inches long, and rehabilitation after the operation is much faster. Patients are encouraged to be active quite soon after surgery (which improves the bone stock), unlike Total Hip Replacement Surgery where the recovery time is over six months. To get started, our surgeons will need to see your x-rays along with the pre-consult form. There is a $130 charge for this service (which is significantly cheaper than return shipping of the xrays by disk or film to multiple doctors). Once the surgeons have reviewed your xrays and pre-consult form, we will inform you as to which surgeons have accepted you as a surgical candidate and some surgical dates. Once you are ready to proceed, let your PlanetHospital Medical Professional know and he or she will assign you a case manager who will take care of firming a surgical date, arranging a phone interview (or email response) from the surgeons of your choice, travel, passport, visas, and more. Post Operative Care A typical hospital stay averages 3 to 7 days. The patient is required to stay in the country where they have their surgery for a minimum of 6 additional days or longer (based on the doctor). Full recovery can take from 6 weeks to 3 months (we have had clients who have gone surfing and horseback riding after surgery). Rehabilitation therapy begins shortly after surgery. Preoperative meetings with a physical therapist will help the patient understand what therapy will be like. Patients should continue with physical therapy until released from their doctor's care.
Much of the initial surgical recovery will happen during the 3 to 7-day hospital stay. The surgeon or attending nurse will administer wound care and give the patient instructions in the hospital with regards to further care and medication usage. The patient should follow the at-home recovery instructions closely to avoid infection and to encourage prompt healing. At home, patients should have assistance during the first few weeks until full mobility returns. |

You should NOT attempt to have Hip Resurfacing if you have ANY of the following conditions:
Poor femoral bone stock Alcohol or Drug Abuse Poor Kidney or liver functions (on a case by case basis) Known allergies to the content of the implants Over the age of 70 (special exceptions may be made). Any tumors or blood disorders (on a case by case basis). The greatest risk that hip resurfacing presents is that the femoral neck may break but this risk is usually eliminated if the surgeon selects the patients correctly in advance. That is why one must always select a very experienced surgeon over a keen surgeon. An experienced surgeon will implant the device in the correct angle and avoiding notching the neck when preparing the femoral head in surgery. The patient should have solid bone stock on the femoral head to make sure the resurfacing prosthesis is held properly and strongly in place. If the patient has a slight Avascular Necrosis (AVN) or cysts it will be at the discretion of the surgeon to treat the patient. The patient should have healthy kidneys since debris or metal ions from the prosthesis may end up in bloodstream (and the patient should have no nickel allergy). The concept of resurfacing has only been around since 1997 therefore there is no long term data. Finally, whether you use a PlanetHospital surgeon or a surgeon in your country, it is important to note that this procedure has a long learning curve and therefore you should only choose a very experienced surgeon for this procedure. General, spinal, or epidural anesthesia will be administered. Choice of Anesthesia depends on the patient and is generally decided after the discussion between the patient and the anesthetists. | | | | | |  | Frequently Asked Questions about Hip Resurfacing Surgery Q: How do I know if I am a good candidate for Hip Resurfacing? A: Only an x-ray will tell us for sure but If you are below the age of 65 (with some exceptions) and have lead a relatively active lifestyle regardless of your hip pain, you might qualify. Fill out our intake form and send us your x-rays and one of our profiled surgeons will get back to you. Q: How long does the surgery take? A: Most of the surgeons on our recommended list take 1-2 hours for one hip and about 3 hours for both hips. The condition of your hip at the time of surgery determines this. Q: How long will it be before I can take a shower or bath? A: You may shower 3 days after surgery if the wound is dry. Keep water exposure to the incision site brief and blot it dry when you get out (do not use a hair dryer to dry the wound). Q: When can I drive a car? A: This is a very difficult question to answer and it depends on the individual. First of all, you need to be off of any narcotic pain medicines (otherwise you are driving under the influence). Can you get in and out of a car without problems? Are your reflexes and strength back to a relative normal? It might be wise to drive in an empty parking lot with a companion to determine if you are absolutely ready. Q: Will I set off metal detectors? A: Yes but the hospital will provide you with a special card that you will present to airport security and others. Need additional cards? No problem, we will contact the doctor on your behalf and get you a replacement for a nominal fee. Q: Should I put any ointment or treatments onto the incision site? A: For the first 2 weeks you should just keep the incision site dry. Typically there will be a ‘wound glue' applied at the time of surgery and / or ‘steri-strips' or ‘butterfly bandages'. These usually begin to come off at about 2 weeks after surgery. At that time you may begin to apply products such as Mederma ointment, Vitamin E ointment or silicone wound coverings. Q: How long is the incision? A: Most incisions are about 9-15 centimeters long (6-9 inches). Q: How long will I be on crutches? A: This depends on the individual. Some patients like to try walking without crutches or walkers within 4-5 days from being discharged but surgeons recommend the use of a cane. Q: How long before I am pain free? A: After surgery, almost all patients notice a huge improvement the moment they take their first step. "The pain is there but not as severe" said one of our clients on ABC Television's NIGHTLINE. Most patients feel a great improvement within two weeks and a relatively pain free life within two months. Q: Can I participate in sports after a Hip Resurfacing surgery? A: Absolutely! In fact that is one of the best advantages of Hip Resurfacing. We have had clients who have been able to ski, skydive, ride horses, and surf, a few months after surgery. The level of activity is an individual matter but if you were active before you should be able to be active again but first talk to your surgeon about his recommendations. Q: When can I resume sexual activities? A: There is no specific time frame. Basically, you can resume such activities when your hip is comfortable enough to allow it. Of course, you should avoid positions that place too much stress on the hip. The therapist can review safe techniques. Q: How long should I keep doing the prescribed post-operative exercises? A: You should continue to exercise until your muscles are pain-free and you can walk without a limp. It is a good idea to continue your exercises as a lifetime commitment to keep your muscles strong. Q: When does the tape come off my wound? A: At the time of surgery tape-like strips may be placed on your incision to protect it. These will eventually come off in one to two weeks, or you may remove them yourself after two weeks. Q: How long will my hip resurfacing last? A: The durability of hip resurfacing is not known as there is no long term data. However, resurfacing prosthesis use the same metals as hip replacement metals which themselves have lasted typically well past 20-25 years. Q: What are the benefits of hip resurfacing compared to hip replacement? A: While conventional Total Hip Replacement Surgery is a good option for an elderly person (those over 70 years old), it's a poor choice for young patients, as it will fail rapidly. The hip resurfacing operation as an alternative to Total Hip Replacement Surgery has three crucial advantages. The first is that no plastic (polyethylene) is used like in conventional hip replacement. Since a anatomical-sized 'metal on metal' bearing is used, it lasts for a very long time. The anatomy and bio-mechanics after resurfacing mimic a normal hip very closely. The second advantage is patients are encouraged to be very active after their surgery, and should play some sport, swim, etc. No activity is restricted, including sitting on the floor, leg crossing, etc. In short, the resurfaced hip behaves like a normal hip, letting patients return to their normal lifestyle. In contrast, after a Total Hip Replacement Surgery the patient has to behave like an elderly person (for whom the Total Hip Replacement Surgery was designed) to be safe from hip dislocation and to prolong the life of a prosthesis. There are other advantages to Hip Resurfacing such as bone stock preservation (no bone is removed in this operation, unlike Total Hip Replacement Surgery where the head and neck of the thighbone is completely removed). It has also been proven that bone stock actually increases after hip resurfacing, due to the restoration of normal bio-mechanics in the hip and proximal femur. The third advantage is that the polyethylene 'wear particles' do not damage the surrounding bone like in conventional hip replacement, and the quality of bone actually improves with time after Hip Resurfacing. This makes a revision solution (if needed at all) a very straightforward surgery, unlike the very complicated revision scenario in a conventional Total Hip Replacement Surgery. The patient usually walks with full weight bearing on the hip the day after the operation, and is usually discharged from the hospital about five to six days after the operation. Patients can resume any work three weeks after the operation and sports can be started six weeks after the operation. |
